But, what happens if your title seems to have grown legs and wandered off?
Luckily, the state of Utah allows you to apply for a duplicate title. Here’s what you’ll need to do to start that process:
You’ll first have to fill out Form TC-123, Application for Duplicate Utah Title. It’s a fairly straightforward document, but keep in mind that if there is any missing or incorrect information in your application, they will have to reject it. Always double check your information!
If there is more than one owner of the vehicle, with names listed on the original title with the word “and” between them, you’ll both have to sign the application.
There is a $6.00 title fee that will need to be paid with your application. The Utah Division of Motor Vehicles asks that you not send cash in the mail, and to make all checks payable to the Utah State Tax Commission (USTC).
Once you’re finished with your duplicate title application and have your check written for the title fee, you can mail it off to the following address:
Division of Motor Vehicles
Mail and Correspondence
P.O. Box 30412
Salt Lake City, UT 84130
Now you’re on your way to receiving a duplicate vehicle title in the mail!
