You'll need the following forms and identification:
- DMV 14
- REG 227
- REG 256 On this form you will write you are requesting a duplicate title only and have moved out of state. This will be required if the vehicle is registered out of state.
- A copy of your out of state license or identification card
A copy of your previous CA license or identification card (if you have it).
